I understand that massage and body treatments are intended for relaxation and therapeutic purposes only. They are not a substitute for medical examination, diagnosis, or treatment. I have disclosed all relevant medical information and will update the therapist of any changes to my health status.

I understand that the therapist reserves the right to end a session if inappropriate behaviour occurs. I acknowledge that any treatment received is at my own risk, and the spa and its staff are not liable for any injury or adverse reaction that may occur as a result of the treatment.

I consent to receive massage and/or body treatments and release the Escape Day Spa & Ferguson Valley Escape and its employees from any liability associated with these services.
